Job Address:
3364 Kolbe Road Suite 209 Lorain, OH 44053 RN Assessor $7,000 Sign On Bonus! RN required
Shift:
3 12-hour shifts per week Full Time, Part Time, and PRN (
Full Time:
3 12-hour shifts, with every other weekend rotation)
Night Shift Hours:
7p-7a The Role Itself Coordinates care with interdisciplinary teams and external providers and updates treatment team on clinical information. Participates in treatment planning, including psychosocial assessments. Provides resource information and "warm-referrals" to community providers for engagement. Completes thorough, timely documentation in compliance with standards. Demonstrates crisis intervention skills and upholds confidentiality. Completes the admission process, including verification of benefits and necessary authorizations.
