Job Description

Generations Home Health is looking for an RN Case Manager! Territory would be Athens County, Meigs County and Washington County. Job Summary Join our dynamic healthcare team as a Registered Nurse (RN) specializing in Home Health, where your expertise will empower patients to achieve optimal health and independence in the comfort of their own homes. In this vital role, you will deliver personalized nursing care, perform comprehensive assessments, and coordinate with multidisciplinary teams to ensure seamless patient experiences. Your energetic approach and compassionate attitude will make a meaningful difference in the lives of individuals across diverse communities, fostering healing, education, and support every step of the way. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans. Administer medications via IV infusion, injections, or other routes while adhering to aseptic techniques and safety protocols. Manage complex wound care procedures such as suturing and ostomy care, ensuring optimal healing and infection prevention. Provide specialized treatments including botulinum toxin injections for therapeutic purposes and assist with ophthalmology-related procedures as needed. Educate patients and their families on disease management, medication adherence, nutrition, and self-care strategies tailored to individual needs. Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate comprehensive home-based care. Maintain meticulous medical documentation using EMR (Electronic Medical Records) systems. Qualifications Valid RN license in good standing with current state requirements. Proven experience in home health nursing or related settings such as hospital medicine, ICU, or critical care environments. Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth for accurate documentation. Strong knowledge of medical terminology, physiology, ICD coding (ICD-10/ICD-9), and diagnostic evaluation processes. Hands-on experience with IV insertion, phlebotomy, laboratory specimen collection/processing, and aseptic technique. Ability to perform complex patient assessments and manage acute pain or trauma cases effectively. Experience working with individuals with disabilities or developmental delays; familiarity with memory care or geriatrics is highly valued. Excellent communication skills for triage calls, patient education, and interdisciplinary collaboration. Certifications such as Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), or specialized wound care credentials are preferred but not mandatory.
