Job Description

Looking for RN - Hospital at Home, All shifts available Step into a dynamic nursing role where hospital-level care meets the comfort of home. As a Registered Nurse (RN) - Hospital at Home , you'll deliver high-acuity, compassionate care to patients outside of a traditional hospital setting-making a real difference where it matters most. This role is perfect for a skilled RN who thrives in a fast-paced, independent environment and values flexibility, autonomy, and meaningful patient connections.
Location:
Westside of Cleveland Area (within 30 miles)
Schedule Options:
PRN, PT, morning, afternoon, evening shifts available | 2 hours shifts available, 4-12 hours shift also available.
Pay Rate:

Flexible shifts (AM, PM, Night options) Deliver hospital-level care in a 1:1 patient setting Strong clinical support and RN leadership team Opportunity to work with advanced home care technology Meaningful patient relationships and outcomes Job Responsibilities Clinical CarePatient Monitoring Perform comprehensive in-home patient assessments Monitor vital signs, symptoms, and overall condition Identify and escalate changes in patient status Skilled Nursing Care Administer medications (oral, IV, injections) Perform wound care, IV therapy, catheter care, and other procedures Execute physician orders and individualized care plans Care Coordination Collaborate with physicians and interdisciplinary teams Coordinate labs, diagnostics, and additional services Ensure seamless care transitions and continuity PatientFamily Education Educate patients and caregivers on disease management and medications Provide guidance on recognizing warning signs and next steps DocumentationCompliance Maintain accurate EMR documentation in real time Follow all regulatory, HIPAA, and company standards Emergency Response Act quickly in urgent situations and initiate appropriate interventions Coordinate hospital transfers when necessary SafetyInfection Control Follow strict infection prevention protocols Maintain a safe and clean care environment Job QualificationsRequirements EducationExperience Active Registered Nurse (RN) license (Ohio) Minimum 1-2 years of RN experience (home health, ER, ICU, or med-surg preferred) Experience with IV therapy, wound care, or acute care strongly preferred Requirements Local candidate within 30 miles of Cleveland area Valid driver's license, reliable transportation, and auto insurance Willingness to travel between patient homes Compliance Current CPR certification Negative TB test (within 12 months) Up-to-date immunizations or exemptions (COVID, Flu, MMR, Hep B) Ability to pass background check and drug screening SkillsCompetencies Strong clinical judgment and critical thinking Ability to work independently in a home setting Excellent communication and patient education skills Tech-savvy (EMR, mobile charting, telehealth tools) Compassionate, adaptable, and patient-focused J oin a Team That Sets the Standard Be part of one of the most trusted names in home care.
