Part-Time Schedule:

Flexible, Monday-Friday with occasional on-call rotation Position Summary Sign on Bonus for FT and PT We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality nursing care to patients in their homes. The Home Health RN will assess patient needs, develop and implement care plans, coordinate with physicians and interdisciplinary teams, and educate patients and families to promote independence and improve health outcomes. Responsibilities Perform comprehensive patient assessments and ongoing evaluations. Develop, implement, and update individualized care plans. Administer medications, treatments, and wound care as prescribed. Monitor patient progress and document care according to agency and regulatory standards. Educate patients and caregivers regarding disease management, medications, and safety. Coordinate care with physicians, therapists, social workers, and other healthcare professionals. Identify changes in patient condition and communicate concerns promptly. Ensure compliance with Medicare, Medicaid, state, and agency regulations. Qualifications Current Registered Nurse (RN) license in the state of practice. Graduate of an accredited nursing program. Minimum of one year of nursing experience; home health experience preferred. Current BLS/CPR certification. Strong assessment, communication, and documentation skills. Valid driver's license, reliable transportation, and proof of auto insurance. Ability to work independently and manage time effectively.
