Experienced Nurse Practitioner (Community Clinic)

Duties Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine appropriate care plans. Manage a wide range of clinical conditions across various age groups, including pediatrics, geriatrics, and adults, with a focus on community health needs. Perform procedures such as suturing, IV infusion, catheterization, and sterile processing while adhering to aseptic techniques and infection control protocols. Administer medications and injections, including botulinum toxin treatments and dermal fillers, ensuring proper documentation in electronic health records (EHR) systems like Epic or eClinicalWorks. Provide health coaching and education on disease prevention, nutrition, medication management, and lifestyle modifications to empower patients for better self-care. Collaborate with multidisciplinary teams on discharge planning, case management, utilization review, and telehealth services to optimize patient outcomes. Document all clinical encounters accurately using

ICD-10/ICD-9

coding standards while maintaining compliance with HIPAA regulations. Skills Proven experience with Epic EMR systems or similar EHR platforms such as Cerner or Athenahealth for efficient documentation. Strong clinical skills in acute pain management, trauma care at Level I/II trauma centers or emergency medicine environments. Expertise in diverse specialties including gastroenterology, infectious disease care, hospice & palliative medicine, geriatrics, behavioral health, and occupational health. Proficiency in performing diagnostic evaluations such as sonography, spinal taps, gastroscopy preparations, or laboratory specimen collection/processing. Skilled in procedures like suturing, phlebotomy, venipuncture, tube feeding management, and airway management techniques including ventilator support. Knowledge of CPT coding for billing purposes alongside ICD coding standards; familiarity with Workers' Compensation law is a plus. Experience working with vulnerable populations such as assisted living residents or individuals with developmental disabilities; familiarity with nursing home or hospital medicine settings is highly valued. Ability to triage effectively in urgent care or outpatient environments while managing complex cases involving acute care or post-acute care scenarios.
