JAG Healthcare Union City is a welcoming 43-bed skilled nursing facility specializing in the care of residents with cognitive deficit disorders , with special attention to individuals with geriatric psychiatry and memory care needs . We are seeking a dependable and team-oriented professional committed to exceptional resident care, clinical accuracy, professionalism, and continuous quality improvement. Key Responsibilities (include but are not limited to): Conduct and coordinate timely MDS assessments, including admission, quarterly, annual, and significant change assessments Collaborate with interdisciplinary team members including nursing, therapy, social services, dietary, and other departments to gather and review assessment information Develop, review, and maintain person-centered care plans aligned with resident needs and regulatory requirements Ensure accurate coding and timely submission of MDS documentation in accordance with CMS, state, and facility guidelines Review and analyze assessment data to support care planning, reimbursement accuracy, and quality improvement initiatives Monitor resident clinical status and communicate relevant changes or concerns to the care team Provide education and support to staff regarding MDS processes, care planning, and compliance standards Maintain knowledge of federal, state, and facility regulations related to MDS documentation and long-term care compliance Skills & Qualifications Current Registered Nurse (RN) license in the State of Ohio required Minimum of two (2) years of nursing experience preferred, ideally in long-term care or skilled nursing Previous MDS coordination, assessment, and care planning experience preferred Strong understanding of Medicare, Medicaid, reimbursement systems, and long-term care regulations preferred Excellent organizational, communication, and critical thinking skills Ability to work collaboratively in an interdisciplinary team environment Strong attention to detail and ability to manage deadlines and regulatory requirements Why Join JAG Healthcare?
