This vital role involves providing comprehensive nursing care to patients in their homes or hospice settings, ensuring comfort, safety, and optimal health outcomes. The ideal candidate will possess extensive clinical expertise, including critical care, emergency medicine, and post-acute care experience, to deliver high-quality patient-centered services. Responsibilities Conduct thorough patient assessments, including vital signs, physiological evaluations, and medical history reviews to develop individualized care plans. Deliver direct nursing care in home health or hospice environments, managing acute and chronic conditions with professionalism and empathy. Coordinate discharge planning, case management, and utilization management to ensure seamless transitions across care settings. Perform diagnostic evaluations and manage complex cases involving infectious diseases, behavioral health issues, or geriatrics. Maintain accurate medical records in compliance with HIPAA regulations while documenting patient progress and interventions. Educate patients and families on medication management, symptom control, and post-discharge care instructions. Collaborate with interdisciplinary teams—including physicians, therapists, social workers—to optimize patient outcomes. Monitor patient responses to treatments and adjust care plans accordingly to ensure safety and effectiveness. Support outpatient clinics or inpatient units as needed, providing specialized nursing expertise in hospital or post-acute settings. Ensure compliance with healthcare standards by adhering to medical terminology protocols and hospital policies. Requirements Valid RN licensure with current registration; additional certifications in critical care or emergency medicine preferred. Extensive clinical experience in hospital environments such as ICU, Level I Trauma Centers, Level II Trauma Centers, or Medical-Surgical units. Proven expertise in post-acute care settings including home health, hospice, outpatient clinics, or inpatient facilities. Strong knowledge of physiology, diagnostic evaluation techniques, and patient assessment procedures. Experience with managed care processes, discharge planning, case management, and utilization management. Familiarity with medical records systems and adherence to HIPAA privacy standards. Excellent communication skills for effective patient service delivery and interdisciplinary collaboration. Ability to handle complex cases involving pediatrics (PICU), geriatrics, behavioral health issues, or infectious diseases. Knowledge of medical terminology relevant to acute care environments; proficiency in vital signs monitoring is essential.
