Job Description

Comfort Keepers is a licensed home care agency that serves seniors and veterans across Oklahoma. Under the supervision of the General Manager and/or Director of Clinical Services, the Registered Nurse or RN Case Manager is responsible for assessing & monitoring patients. RN performs initial patient assessments & home safety evaluations, designs, enters & implements the plans of care, and conducts periodic re-evaluations (every 3-6 mos). The RN interacts with clients, their spouses, family members & significant others, and coordinates with other healthcare providers... all while maintaining standards of professional conduct. RN provides case management AND customer service, and provides advice & information on health & wellness, medications and fall prevention.
Specific Nursing Duties:
Performs intake calls and helps with follow-up with potential clients ("prospects") Conducts initial assessments / in-home visits, performs re-assessments, home safety evaluations, determines client needs, develops and communicates a plan of care to the family. Conducts regular on-site supervisory visits to evaluate service quality and obtain feedback on Comfort Keeper caregivers. Regularly re-evaluates client plans of care to ensure client needs are met. Documents changes in condition. Informs clients of their rights and responsibilities and respects these rights and responsibilities. Ensures ongoing coordination of care between the client, caregiver, primary care physician, and other Comfort Keepers office staff. Provides skills assessments for new hires (CNAs) and hands-on training for existing caregiving staff. Participates in Quality Assessment Process Improvement (QAPI) activities. Documents incidents, injuries, complaints, infections. May be involved in orientation and onboarding of new caregiving staff along with HR team. Data entry of new clients into Wellsky, i.e. electronic records/patient charts, incl. notes from visits, calls.
Compensation:
  • Earn $100-150 flat fee for each INITIAL assessment (usually 60-90 minutes in-home) plus data entry of plan (ADLs)/medication details… higher amount for 24-36hr response rate
  • PDN visit at $50/hr rate
  • Earn $60-80 flat fee for each re-assessment (done every 6mos, typically 30-45 minutes)
  • Make $38-45 per hour (base wage) for documentation and other office duties requested, such as CNA skills assessments, appraisal work, coaching
  • Receive mileage reimbursement for use of personal vehicle ($0.58 / mile)
  • Paid $8/hr for travel time (there are rural cases) Why Comfort Keepers Home Care?

Required Skills:
Proven experience as a registered nurse (RN) caring for the elderly Excellent knowledge of home health, private duty nursing (PDN) care methods and procedures Excellent knowledge of geriatrics (diseases and chronic conditions affecting the elderly) In-depth knowledge of health and safety guidelines and procedures (infection control, PPE, PHI, etc.) and willingness to follow them at all times A team player with excellent communication and interpersonal skills Responsible and compassionate Outstanding organizational and multi-tasking skills Possess excellent problem-solving skills Minimum Job Qualifications /
Requirements:
Valid nursing license (RN) 3 years work experience in home health, hospice, rehab, or home care
