DESCRIPTION OF WORK
Part-time / As needed Provides professional nursing care to the agency's homecare clients as prescribed by the physician and in compliance with the State Registered Nurse Practice Act and the agency policies and procedures. May also provide professional nursing services in an institutional setting within the policies and procedures of that institution.
Supervision Received:
Works under the general direction of the DCS. In an office where there is not a DCS, works under the general direction of the
Administrator/Branch Supervisor Supervision Exercised:
May supervise services provided by home health aides.
QUALIFICATIONS FOR EMPLOYMENT
Currently licensed as a registered nurse under the laws of the state (s) in which they intend to practice. Preference may be made to nurses with previous home health experience. Recent experience in direct patient care in an institution or home health setting is preferred.
NOTE:
Evidence of successful completion of an approved nursing skills refresher program may be accepted in lieu of this experience. Must be flexible and able to adjust to changing tasks frequently. Requires good hearing to assess situations quickly and respond appropriately. Good color vision essential to determine subtle changes in patient's skin color and to rapidly recognize medication and color coded supplies. Light to moderate physical activity required, including possible lifting and turning of adult clients. Light to moderate degrees of stress incurred in daily employment. Must have valid driver's license, reliable vehicle, maintain valid auto liability insurance, and be able to drive throughout service area under a variety of weather conditions. Contact with a variety of housing conditions required. Must be able to maintain concentration during frequent interruptions.
TYPICAL DUTIES AND RESPONSIBILITIES
1. Provide skilled nursing patient care or service within the established plan of treatment. 2. May supervise home health aides on a routine basis to accomplish the plan of treatment which has been developed. 3. Assesses patient physical, psychological, social and environmental status. 4. Reports pertinent observations and reactions regarding changes in patient status to the appropriate team person and provides follow-up to assure resolution of any identified problem. 5. Confers with other home health team members and participates regularly in patient care conferences to facilitate coordination of patient care. 6. Maintains acceptable standards of practice as evidenced by quality assurance activities. 7. Records observations accurately and concisely. 8. Maintains and completes patient's medical record in accordance with agency standards. 9. Completes and submits complete patient/client records in a timely manner. 10. Maintains patient confidentiality at all times. 11. Plans and implements nursing interventions to meet identified patient physical, psychological, social, and environmental needs. 12. Evaluates patient's response to approved plan of treatment. 13. Administers medication and performs treatments to assigned patients. 14. Participates in on-call, holiday, and weekend rotation, if appropriate. 15. Participates in-service education programs to learn new treatments, procedures, developmental skills, etc. 16. Shares expanded knowledge and skills with other staff members. 17. Interprets agency philosophy and policies to patients, families and other groups. 18. Utilizes appropriate community resources, identifies new resources, and shares. 9. Utilizes agency resources efficiently. 20. Provides own transportation that is in good working order.
ADDITIONAL DUTIES AND RESPONSIBILITIES MAY INCLUDE
1. Analyzes patient clinical and environmental picture and identifies actual and potential needs. 2. Plans, organizes and coordinates client services for identified home care needs. 3. Assists with orientation and training of new staff. 4. Provide on-site training to staff nurses and aides that are new to home care. 5. Provide in-service for compliance to documentation standards. 6. Assists with writing and updating quality assurance plans, policies and procedures, including in-services and training. 7 . Acts as on-site quality assurance representative to facilitate coordination and communication of quality assurance philosophy, standards, goals, and policies and procedures, including in-service and training. 8. Reviews all client documentation to ensure payability prior to submission of documentation. 9. Returns non-complaint documentation to staff nurses for correction, 10. Reviews documented client care denied by payer for lack of medical necessity. 11. Assists with formulation and implementation of all appropriate appeal process. 12.
