Licensed Practical Nurse (LPN) - Skilled Nursing /
Long-Term Care Position Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing and long-term care team. The LPN will provide high-quality, resident-centered care while collaborating with an interdisciplinary team to support the health, comfort, and well-being of our residents.
Responsibilities:
Provide direct nursing care to residents, including medication administration, wound care, treatments, and vital sign monitoring Develop, update, and implement individualized resident care plans in collaboration with the interdisciplinary care team Perform and oversee routine resident care needs, including assessments, bathing, toileting, and activities of daily living support Monitor resident conditions and promptly report changes in health status Maintain accurate, timely, and thorough documentation of resident care, treatments, and progress Communicate effectively with residents, families, providers, and team members to promote coordinated care Support infection prevention practices and facility standards of care Participate in quality improvement initiatives focused on improving resident outcomes, safety, and satisfaction Promote a respectful, compassionate, and supportive environment for residents
Requirements:
Current Licensed Practical Nurse (LPN) license in the State of Oklahoma Previous experience in skilled nursing, long-term care, or geriatric nursing preferred Strong clinical assessment and communication skills Ability to work collaboratively as part of a multidisciplinary team Commitment to providing compassionate, resident-focused care Join our team and make a difference in the lives of our residents every day!
