As an RN Field, you will utilize your critical care expertise and broad clinical knowledge to assess patient needs, coordinate care plans, and ensure optimal health outcomes. Responsibilities Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine immediate needs and ongoing care plans. Provide expert nursing care in various environments such as hospitals, outpatient clinics, inpatient units, and post-acute settings. Collaborate with multidisciplinary teams to develop and implement individualized discharge planning and case management strategies. Manage complex cases involving critical care, trauma, infectious disease, geriatrics, behavioral health, pediatrics, or medical-surgical needs with precision and compassion. Perform diagnostic evaluations, monitor patient responses to treatments, and adjust interventions accordingly to optimize recovery. Ensure compliance with HIPAA regulations and maintain accurate medical records while safeguarding patient confidentiality. Support utilization management by coordinating intake processes, facilitating diagnostic assessments, and managing patient service workflows efficiently. Qualifications Valid RN license with current registration in the applicable state or jurisdiction.. Familiarity with hospital records systems and medical documentation standards such as electronic health records (EHR). Excellent communication skills with the ability to perform diagnostic evaluation and discharge planning effectively.
