Job Description

Emerald Hospice is currently seeking a full time RN Case Manager. The RN Hospice Case Manager is responsible for coordinating all aspects of the interdisciplinary care and services provided to hospice patients and their families. The ideal candidate collaborates with the patient and/or caregiver(s) and the other members of the Hospice Interdisciplinary Group to develop and implement the Plan of Care for the patient. He/She will have all basic nursing skills, including but not limited to nursing assessments, current symptom management knowledge, patient/caregiver teaching, and documentation. Competitive Pay. Pay negotiable, based on experience. Requirements Graduate of an accredited RN program Current state license as a Registered Nurse Minimum of one year of Hospice experience preferred Valid driver's license and reliable transportation Participate in on-call as assigned Emerald Hospice is an Equal Opportunity Employer.
