Pay:
Hourly, PG13 Weekly earned wage access is an option for this position Job Purpose or Objective(s): As the Case Manager RN at the Diabetes Wellness Center, you will provide case management services and coordinate the delivery of health care for assigned patients. You will supervise other members of the health care team. You will report to the Manager or Director over the department.
Primary Tasks:
1. Assess and evaluate patient needs to prepare and implement treatment and case management plans. 2. Confer with health team members to discuss and evaluate patient information for planning, modifying, and coordinating treatment programs. 3. Explain procedures and treatments to patient to gain cooperation, understanding, and allay apprehension. 4. Administer prescribed medications and treatments following physician orders. 5. Prepare and maintain written case records and reports. 6. Manage cases to ensure that follow-up care is provided. 7. Coordinate appointments for consultations, referrals, and tests. 8. Facilitate communication between care providers and provide information and education to patients. 9. Assume responsibilities of the Unit Manager RN when necessary. 10. You will perform other responsibilities as may be assigned.
Required Education and Experience:
  • Associate degree in nursing
  • Current RN licensure with the Oklahoma State Board of Nursing or Multi-State RN License
  • Current BLS certification
  • Associate degree in nursing
