Participating actively in meeting the goals and objectives of the hospice organization, the RN collaborates and communicates with members of the interdisciplinary team. The RN demonstrates an ongoing commitment to his/her own professional growth. Role Expectations
Essential Job Functions and Responsibilities:
Demonstrates the ability to transfer scientific knowledge from social, biological and physical sciences in applying the nursing process. Demonstrates knowledge and competency in performing baseline-nursing skills. Maintains clinical competencies according to Home Health & Hospice standards. Administers all medications and treatments according to Home Health & Hospice policy and procedure, accepted standards of care and Home Health & Hospice guidelines. Communicates effectively with patients, families and physicians. Performs accurate assessments, develops and implements a written Plan of Care with the patient, following physician's orders and according to Home Health & Hospice guidelines. Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change. Documents patient status in an accurate, timely and concise manner as required by Home Health & Hospice policy. Addresses patient needs and problems, nursing intervention and patient response, safety interventions and education and teaching. Demonstrates organization and leadership abilities by effective use of time and personnel through supervision and teaching. Effectively establishes priorities and delegates responsibilities appropriately when supervising the LPN and the aide. Maintains a committed and cooperative attitude with staffing, promoting teamwork and harmony by readily assisting other staff and patients. Participates in case conferences/team meetings. Make visits as scheduled and participates in on-call rotation as required. Adherence to

Adheres to SC@Home policies and procedures. Clarifies with manager deviation from stated policies and procedures. Demonstrates appropriate resource utilization for meeting patient care needs. Contributes to a working environment that encourages collaboration and communication. Demonstrates accountability for own decisions and actions.
Communication and Interpersonal Relationship Skills:
Maintains a friendly yet professional demeanor when interacting with the patient, caregiver and family, as well as, with members of the interdisciplinary team and office support staff. Respects the professional boundaries of communicating personal information with patient and caregiver. Utilizes effective communication skills both verbal and nonverbal with the patient/family, always showing courtesy and respect for the patient's rights. Serves as patient/family advocate. Initiates and documents follow up to patient/family needs. Performs job with enthusiasm and a positive approach. Utilizes courteous and standardized approaches to phone communication. Maintains confidentiality of privileged information and supports the patient's right to privacy. Orients new personnel to their role as requested by manager. Acts as a resource person to other staff members as needed. Utilizes guidelines governing documentation when completing nursing notes. Able to function independently. Has strong decision making and problem solving skills. Gives concise, orderly, and thorough patient report to the Physician whenever changes in a patientcaregiver's situation occurs. Contributes to the discussion of assigned patients during the interdisciplinary team meeting.
Continuing Education:
Identifies and describes own learning needs. Displays growth in knowledge and skill required to deliver care to complex patient/family situations and current knowledge base related to aspects of palliative care, death and dying. Attends home health & hospice-based education programs, including mandatory in-service programs.
Hours Worked:
Hours worked are based on the assigned patients' needs. On-call on rotation basis including holidays.
Supervisor:
Nurse Supervisor - Overseeing day-to-day operations of IDT team and provision of patient care. Overseeing job performance, professional development, and specific role responsibilities.
Education:
Education:
Graduate of an accredited school of nursing.
Experience:
2 years experience in nursing, preferred 1 year in home health &/or hospice.
License and Qualifications:
Valid Oklahoma registered nurse license in good standing. Current CPR Valid driver's license. Current automobile insurance. Reliable vehicle.
Physical Demands:
Able to sit and/or stand for up to 2 hours per day Walk 3-4 times per day and do so rapidly when necessary Frequently lift up to 50 lbs Assist with transfer, lifting, and pushing a wheelchair Push/pull with up to 50 lbs with force Climb stairs quickly Use Arjo, sit to stand/hoyer lift when appropriate to assist residents from the floor Must be able to occasionally balance, climb, crawl, crouch, kneel, stand, stoop, bend, and twist at the waist, reach abovebelow shoulder heights, frequently grasp and handle paperwork and diagnostic tools. Daily use of computer, telephone, and other diagnostic equipment.
