Job Description
Position Purpose / Summary The Outpatient Infusion Registered Nurse (RN) provides specialized, evidence-based nursing care to patients requiring intravenous (IV) therapies in an outpatient setting. This role is responsible for the assessment, planning, implementation, and evaluation of infusion therapies, including medications, biologics, blood products, and hydration. The RN functions as a clinical expert in vascular access and infusion therapy, ensuring safe administration, monitoring for adverse reactions, and patient education across the continuum of care. This position plays a key role in developing and sustaining a PICC line and vascular access program within a Critical Access Hospital (CAH), supporting both outpatient and inpatient services. The RN works collaboratively with providers, pharmacy, and interdisciplinary teams to ensure high-quality, patient-centered care consistent with organizational standards, regulatory requirements, and the ONA collective bargaining agreement.
Requirements Education and Experience Required:
Graduate of an accredited School of Nursing (ADN or BSN) Minimum 2-3 years of nursing experience Demonstrated competency in IV therapy and medication administration Experience with electronic health record (EHR) documentationPreferred:
Critical care, emergency department, or acute care experience strongly preferred Prior experience in infusion therapy, oncology, or vascular access Experience with PICC line insertion and/or maintenance Experience in rural or Critical Access Hospital settings Experience with care coordination and patient education in outpatient settings License / Certification Requirements Current Oregon RN license in good standing BLS (Basic Life Support) required ACLS (Advanced Cardiovascular Life Support) required ONS Chemotherapy/Biotherapy Certification (if applicable to services provided) Certified Registered Nurse Infusion (CRNI®) or equivalent - required within 1 year of hire PICC insertion certification (or willingness to obtain within 1 year) Core Duties and Responsibilities Clinical Care Perform comprehensive patient assessments prior to infusion therapy, including history, allergies, and risk factors Initiate and maintain peripheral IV lines and central venous access devices (CVADs), including PICCs and ports Administer IV medications, fluids, blood products, and specialty infusions per provider order Monitor patients during and after infusion for adverse reactions, complications, or changes in condition Respond appropriately to infusion-related emergencies (e.g., anaphylaxis, extravasation) Maintain strict adherence to infection prevention and aseptic technique Vascular Access / PICC Program Development Assist in the development, implementation, and evaluation of a PICC line program Participate in establishing policies, procedures, and competency validation for vascular access Perform PICC line insertion (if credentialed) and provide maintenance and troubleshooting Educate staff on vascular access best practices and device management Collaborate with leadership and Infection prevention to track quality metrics (CLABSI, complications, utilization) Care Coordination & Interdisciplinary Collaboration Collaborate closely with providers and pharmacy/pharmacists to ensure safe preparation, verification, and administration of infusion therapies Participate in medication reconciliation and verification processes in alignment with pharmacy protocols Coordinate care with interdisciplinary team members to ensure safe and effective treatment plans Communicate patient status, therapy response, and concerns in a timely manner Patient Education & Community Outreach Educate patients and families on infusion therapies, potential side effects, and home care Provide instruction on central line care and complication prevention Participate in community outreach and education efforts to promote available infusion services and improve access to care in the rural community Support organizational initiatives aimed at improving population health and service awareness Documentation & Compliance Accurately document all patient care, medications, and responses in the EHR Ensure compliance with regulatory requirements (CMS, Joint Commission, state regulations) Maintain knowledge of current evidence-based practices in infusion therapy Participate in quality improvement initiatives Operational & Organizational Responsibilities Manage daily patient schedules, prioritization, and workflow in an outpatient setting Maintain inventory and ensure availability of infusion supplies and medications Demonstrate strong organizational and time-management skills in a fast-paced environment Participate in clinic operations, including coordination of referrals and scheduling support as needed Department & Organizational Support Assist in other patient care units (e.g., Med/Surg, Emergency Department, or other departments) within scope of practice as staffing and schedule allow, in alignment with hospital needs and ONA contract provisions Attend and actively participate in scheduled staff meetings, trainings, and competency reviews Support hospital-wide initiatives, process improvement efforts, and interdisciplinary collaboration Maintain flexibility and adaptability in a Critical Access Hospital environment where cross-training and teamwork are essential Skills and Competencies Advanced IV insertion and vascular access skills Strong critical thinking and clinical judgment Ability to recognize and respond to acute patient changes Excellent communication and patient education skills High attention to detail and medication safety Strong organizational and multitasking abilities Ability to work independently with minimal supervision Working Conditions Outpatient clinical environment within a Critical Access Hospital Monday-Friday schedule;
