Job Summary:

We are seeking a dedicated Registered Nurse (RN) specializing in Home Health care to join our dynamic healthcare team. This role involves providing comprehensive nursing services to patients in their homes, ensuring high-quality, personalized care that promotes recovery, health maintenance, and disease management.

Roles & Responsibilities:

Conduct thorough patient assessments, including vital signs, physical examinations, and review of medical histories to determine appropriate care plans. Deliver direct nursing care in the home setting, managing acute and chronic conditions with precision and compassion. Coordinate discharge planning and facilitate smooth transitions from hospital or clinic to home-based care. Monitor patient progress and adjust treatment plans accordingly, utilizing clinical judgment informed by physiology knowledge and medical terminology expertise. Collaborate with interdisciplinary teams—including physicians, case managers, and therapists—to optimize patient outcomes. RN01

Required Qualifications:

Min 1 year of RN Home health experience required

MUST HAVE BOTH OR AND WA LICENSURE BLS
