Job Description
Overview We are seeking a dedicated and compassionate MAT Coordinator to join our team and provide quality care to our patients at the Yamhill County Correctional Facility. As a vital member of our medical staff, you will provide high-quality patient care in a correctional setting. Your expertise will play a crucial role in assessing patient needs, implementing care plans, and collaborating with multidisciplinary teams to ensure optimal health outcomes.
Responsibilities:
Oversee daily operations of the MAT program including scheduling, medication administration, documentation, and coordination of care. Serve as the primary liaison between medical staff, mental health providers, custody staff, and community partners to ensure a seamless MAT process. Assist with program expansion, audits, and compliance reviews to meet NCCHC and Oregon state requirements. Administer MAT medications (e.g., buprenorphine, methadone, naltrexone)in accordance with provider orders and facility protocols. Conduct intake screenings and assessments for MAT eligibility in collaboration with providers. Provide education to AICs on MAT, medication adherence, and recovery support. Monitor and document patient response to medications, side effects, and treatment outcomes. Coordinate discharge planning for MAT participants to ensure seamless continuity of care upon release. Establish and maintain referral relationships with community MAT providers and recovery programs. Ensure proper release-of-information and documentation are completed for continuity of treatment. Work alongside nursing and custody staff to ensure safe and effective medication delivery practices. Train nursing staff in MAT-related protocols and support new staffing in understanding MAT workflows. Collaborate with the medical provider and site leadership to evaluate and improve MAT program delivery. Communicates the need for special housing to appropriate medical and security personnel. Thoroughly documents all patient encounters in electronic or paper medical records in accordance with MedHealth Policies and Procedures, and within state, local, federal guidelines. Attends special needs and Medical Administrative Committee meetings with security personnel and mandatory monthly staff meetings with medical staff. Other duties as necessary.Qualifications:
Valid, unrestricted LPN or RN license in the state of Oregon Basic Life Support (BLS) for healthcare providers through American Red Cross or American Heart Association MAT experience - preferred Corrections experience - highly desiredJob Details:
Full-Time, Monday-Friday:
