Job Description

Job Summary We are seeking a compassionate and skilled Registered Nurse for our Home Health division. The ideal candidate will provide high-quality patient care in the comfort of patients' homes, ensuring their health and well-being through comprehensive assessments, individualized care plans, and effective communication with healthcare teams. This role requires a strong background in nursing practices and a commitment to delivering exceptional care to diverse patient populations. Duties Conduct thorough patient assessments to develop and implement personalized care plans. Administer medications and treatments as prescribed, including IV infusions and injections. Monitor vital signs and document patient progress accurately in electronic health records (EHR). Provide education to patients and families regarding health management, medication administration, and home care techniques. Collaborate with interdisciplinary teams to coordinate care for patients with complex medical needs. Assist with procedures such as catheterization, tube feeding, and wound care. Ensure compliance with HIPAA regulations and maintain confidentiality of patient information. Participate in discharge planning and case management for continuity of care. Provide support for patients with developmental disabilities, memory care needs, or chronic conditions. Requirements Current Registered Nurse (RN) license in the state of practice. Experience in home health or community-based nursing preferred. Proficient in using EMR systems such as Epic or Cerner for documentation. Familiarity with ICD-10 coding and NCQA standards is advantageous. Critical care experience or experience in Level I or II trauma centers is a plus. Knowledge of medical terminology, anatomy, physiology, and infection control practices. Skills in patient assessment, triage, and acute pain management are essential. Experience working with diverse populations including geriatrics, pediatrics, or those requiring hospice & palliative medicine is highly desirable. Ability to perform essential nursing skills such as phlebotomy, venipuncture, suturing, and airway management. Strong communication skills to effectively educate patients and collaborate with healthcare teams.
