Job Summary:

We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. This vital role involves coordinating patient care, managing case plans, and ensuring seamless communication across multidisciplinary teams. The ideal candidate will possess extensive clinical expertise, including critical care, trauma, and hospital medicine experience, to deliver comprehensive case management services.

Roles & Responsibilities:

Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs. Develop individualized case management plans that incorporate diagnostic evaluations, discharge planning, and post-acute care strategies. Coordinate inpatient and outpatient services across various settings such as hospital floors, clinics, emergency rooms, and specialty units like PICU or Geriatrics. Collaborate with multidisciplinary teams—including physicians, social workers, therapists, and specialists—to optimize patient care pathways. RN01

Required Qualifications:

Min 1 year of RN Case Manager experience required

$2,350.00 - $2,766.00 per week
