  • Full-time Position Summary The Home Health QA Review Nurse is responsible for reviewing all Start of Care and discharge OASIS assessments, related documentation, and associated plans of care for accuracy, completeness, consistency, and regulatory compliance.
This position reports directly to the Clinical Director and serves as a centralized quality resource that provides leadership with objective visibility into documentation performance across Clinical Managers and field staff, while also supporting Clinical Managers through feedback, education, and timely issue resolution. Position purpose This role exists to ensure that Start of Care and discharge documentation is accurate, complete, and defensible, and to give the Clinical Director a reliable documentation-based view of operational and clinical performance across the management team. At the same time, the QA Review Nurse functions as a collaborative partner to Clinical Managers by helping identify issues early, reinforcing documentation standards, and supporting consistent application of agency policies and home health requirements. Key responsibilities Review all Start of Care and discharge OASIS assessments, related visit documentation, and plans of care for clinical accuracy, internal consistency, completeness, and compliance with agency standards and home health regulatory requirements. Report directly to the Clinical Director and provide an objective quality checkpoint for documentation review across branches, clinicians, and Clinical Managers. Support the Clinical Director in overseeing Clinical Managers by identifying documentation trends, recurring deficiencies, completion issues, and compliance risks that may require coaching, follow-up, or corrective action. Support Clinical Managers by serving as a resource for documentation clarification, OASIS accuracy, discharge accuracy, and plan-of-care alignment, helping them resolve issues before they become larger compliance or billing problems. Communicate review findings promptly and professionally to Clinical Managers and clinicians, with clear guidance for corrections, completion, and improvement. Track patterns in documentation quality at Start of Care and discharge, including missed items, unsupported responses, conflicting documentation, and inconsistencies that may affect outcomes, reimbursement, or audit readiness. Collaborate with Clinical Managers to strengthen documentation practices, improve consistency across teams, and support a culture of accountability and success rather than only retrospective error correction. Escalate significant or repeated concerns to the Clinical Director and provide useful trend reporting that helps leadership manage performance with facts, consistency, and fairness. Role statement The QA Review Nurse serves as both a quality safeguard for the Clinical Director and a documentation support partner for Clinical Managers, helping ensure agency success through accurate and timely clinical records
