Job Overview We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The ideal candidate will have prior Home Health experience, strong assessment skills, and the ability to work independently while managing a diverse patient caseload. This role focuses on delivering high-quality patient-centered care, promoting recovery, and supporting patients in maintaining their independence. Key Responsibilities Perform Start of Care (SOC), admission, recertification, resumption of care, routine visits, and discharge assessments. Develop and implement individualized care plans based on patient needs. Provide wound care and monitor healing progress. Administer medications, injections, IV infusions, and other prescribed treatments. Conduct medication reconciliation and patient medication management. Assess and manage pain using appropriate pain scales and interventions. Educate patients and families on disease management, medication administration, and home safety. Maintain accurate documentation using Epic and OASIS systems. Follow infection control and patient safety protocols. Collaborate with physicians, therapists, case managers, and other healthcare professionals. Travel between patient homes within the assigned coverage area. RN01 Required Qualifications Minimum of 1 year of recent Skilled Home Health RN experience. Active Oregon state Registered Nurse (RN) License. Current BLS (Basic Life Support) Certification. Epic EMR experience required. Valid driver's license and reliable personal transportation. Strong assessment, documentation, and communication skills.
