Job Description

Overview Join our dedicated healthcare team as a Registered Nurse (RN) specializing in Home Health, where your expertise will directly impact patients' lives by delivering compassionate, comprehensive care in their own homes. This dynamic role offers the opportunity to utilize your critical care, ICU, emergency medicine, and post-acute care experience to promote healing, independence, and well-being. As an RN Home Health professional, you will coordinate patient assessments, develop personalized care plans, and ensure seamless communication among multidisciplinary teams. Your proactive approach and clinical skills will foster positive health outcomes while empowering patients on their recovery journey. Responsibilities Conduct thorough patient assessments including vital signs, physiological evaluations, and medical history reviews to determine individual care needs. Develop and implement tailored home health care plans aligned with physician orders and patient goals. Monitor patient progress through regular visits, adjusting treatments as necessary while maintaining detailed medical records in compliance with HIPAA regulations. Collaborate with physicians, case managers, and other healthcare professionals to coordinate discharge planning and post-acute care strategies. Provide education on medication management, infection control, behavioral health support, and chronic disease management to patients and their families. Perform diagnostic evaluations and manage complex cases involving infectious diseases, geriatrics, pediatrics, or behavioral health conditions. Ensure safety standards are upheld during all visits by adhering to hospital protocols and utilizing your knowledge of hospital medicine and emergency procedures. Qualifications Valid RN licensure with current state certification required; additional certifications in critical care or trauma nursing are highly desirable. Proven experience in ICU, emergency room settings, or Level I/II trauma centers demonstrating advanced patient assessment skills. Strong background in hospital medicine, post-acute care, outpatient clinics or managed care environments. Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and utilization management practices. Experience working with diverse populations including geriatrics, pediatrics (PICU), or behavioral health patients is preferred. Ability to perform comprehensive patient evaluations and discharge planning efficiently while managing multiple priorities. Excellent communication skills coupled with a commitment to patient service excellence and adherence to HIPAA guidelines.
