Job Description

Private duty CNA or LPN needed for a 70 year old disabled dementia-like female in her home. Certification must be up to date Certification must be a CNA, LPN, or RN Nurse must pass background checks Nurse must provide 3 references and work history.
About the patient:
Patient has left side difficulty Patient needs assistance with basic hygiene, administering meds, and meals. The care is provided in the patients home in a wheelchair friendly suite of the house. Patient has memory problems and is often confused Patient has sight problems Patient needs assistance transferring from wheelchair to bed. There is an electric Hoyer lift on hand but a nurse assisted pivot transfer is preferred to keep the patient's legs strong.
Job hours:
Hours can vary and will be chosen based on applicant's availability. Weekdays the typical hours needed are 3:30pm - 9:30pm. Weekends 9/10am-10/11pm with another nurse splitting those weekend hours if desired. Number of hours will depend on applicant's availability. 12-31 hours are available PRN is also available Non-major holidays are shared/rotated with the other nurses (I.e. Memorial day, July 4th, Labor Day etc) Job requirements: Administering medications Restocking supplies Basic hygiene Some basic physical therapy movements to maintain strength Help with basic household chores Making the patient meals with food provided. Attend periodic appointments with the patient, with reimbursement for gas.
Pay Rate:
$24/hour for CNA plus a $3/hour differential pay daily for 5pm-11pm hours. $28/hour for LPN plus a $3/hour differential pay daily for 5pm-11pm hours. Time and 1/2 on holidays Applicant should be prepared to do a pivot transfer with the patient during the interview. This is to make sure that the applicant is capable of transferring the patient on their own. The transfer is not expected to be perfect since it would be the first time, but show potential capability.
