Job Description

Overview We are seeking a compassionate and highly skilled Home Health Registered Nurse to provide exceptional patient care in a home and community setting. The ideal candidate will have extensive experience in various clinical environments, including acute care, geriatrics, pediatrics, and behavioral health. This role requires a strong understanding of medical documentation, home Care Home Base systems, and adherence to HIPAA regulations. The Home Health RN will play a vital role in patient assessment, care planning, medication administration, and education to promote optimal health outcomes. Experience with diverse medical procedures such as IV infusion, catheter care, wound management, and infection control is essential. Candidates with specialized skills in hospice & palliative medicine, trauma medicine, or working with special populations like developmental disabilities are highly valued. Responsibilities Conduct comprehensive patient assessments and vital signs monitoring in home settings Develop and implement individualized care plans aligned with physician orders and patient needs Administer medications, injections, IV infusions, and perform catheterization and wound care following aseptic techniques Manage complex medical conditions including acute pain management, respiratory support (ventilators), and infection control protocols Document all patient interactions accurately using Home Care Home Base EMR/EHR system. Educate patients and families on disease management, medication adherence, nutrition, and self-care techniques Coordinate discharge planning and collaborate with multidisciplinary teams for post-acute care or hospice services Ensure compliance with HIPAA regulations and maintain confidentiality of patient records Provide specialized care for diverse populations including geriatrics (memory care, Alzheimer's/dementia care), pediatrics (neonatal ventilator, toddler care), and individuals with disabilities or developmental challenges Participate in ongoing professional development to stay current with medical standards such as NCQA standards, ICD coding (ICD-9/ICD-10), CPT coding, and infection control practices Skills Proven experience in home health nursing across various specialties such as medical-surgical, ICU/CCU/PICU/trauma centers or outpatient clinics Proficiency with home care home base Strong knowledge of physiology, anatomy, pharmacology (medication administration), and diagnostic evaluation techniques Ability to perform physical examinations and diagnostic assessments effectively Skilled in IV insertion/infusion therapy, phlebotomy, specimen collection/processing, and laboratory experience Expertise in managing complex cases involving ventilators, catheters (ostomy), NG tubes (gastric lavage), or tube feeding Excellent communication skills for patient education and interdisciplinary collaboration Knowledge of infection control protocols and aseptic techniques to prevent healthcare-associated infections Familiarity with Workers' Compensation law is advantageous for managing occupational health cases Ability to work independently while maintaining high standards of patient safety and quality care This position offers an opportunity to make a meaningful difference in patients' lives by providing comprehensive home health nursing services.
