Job Description

Job Overview We are seeking a dynamic and compassionate Nurse Practitioner specializing in Geriatric Behavioral Health to join our dedicated healthcare team. In this vital role, you will provide advanced clinical care tailored to the unique mental health needs of older adults, including those with dementia, Alzheimer's, and other age-related cognitive conditions. Your expertise will help improve quality of life for seniors by delivering comprehensive assessments, evidence-based interventions, and collaborative care plans. Responsibilities Conduct thorough psychiatric and medical assessments for geriatric patients, including mental health evaluations and medication reviews. Develop and implement individualized treatment plans that incorporate behavioral health strategies, medication management, and patient education. Collaborate closely with interdisciplinary teams including physicians, social workers, therapists, and family members to coordinate holistic care. Administer or oversee administration of treatments such as Botulinum toxin therapy for specific conditions when appropriate. Manage complex cases involving dementia, memory care, and behavioral disturbances, ensuring safety and optimal outcomes. Utilize electronic health record (EHR) systems such as Medsphere document patient encounters accurately and efficiently. Provide telehealth services to expand access to care for seniors in various settings, including assisted living facilities and home care environments. Skills Proven experience working with older adults in geriatric or behavioral health settings; experience with dementia and Alzheimer's care is highly valued. Strong knowledge of medical documentation standards, CPT coding, ICD-10 coding systems, and HIPAA compliance. Skilled in performing clinical procedures such as venipuncture, IV infusion, suturing, catheterization, spinal taps, and sterile processing techniques. Ability to perform comprehensive patient assessments including vital signs, physical examinations, and physiology knowledge. Experience managing acute pain cases, medication administration including psychopharmacology, and utilizing telehealth platforms effectively. Knowledge of nursing home protocols, assisted living environments, post-acute care processes, and discharge planning procedures.
