Job Description

Hospice Registered Nurse Case Manager (RNCM) Manageable caseloads, flexibility, and meaningful hospice work that allows you to focus on quality patient care. Overview The Care Team is seeking a Registered Nurse Case Manager to join our growing team! The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team. EMR HCHB
(Homecare Homebase) Participate in interdisciplinary team meetings and care planning Support after-hours patient needs as part of an on-call rotation when applicable Qualifications Active RN License required Hospice nursing experience preferred Experience with Homecare Homebase (HCHB) preferred Strong assessment, communication, and care coordination skills Schedule & Work-Life Balance Monday-Friday, 8:00 AM - 4:30 PM On-call opportunities available with additional compensation Some locations offer dedicated on-call teams, resulting in backup coverage only
Key Responsibilities:
Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care. Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB). Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care. Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes. Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
