Role:

This is a Skilled Case Management position—no clinical care required. Schedule and perform Q60 day HHA Supervisory/Recertification visits Schedule and perform Start of Care assessment visits Ongoing Case Management for client and HHA oversight Evaluate the client status and the skills of staff members Complete timely and accurate clinical assessments within 3 days of the visit Complete timely and accurate 485/ Plan of Care for patients within 3 days of the visit Provides patient, family, or caregiver education The role is Office based out of our Bethlehem Location with local travel, typically 3-4 home visits per day for brief assessments. Schedule Monday through Thursday 2:00 PM-7:00 PM NO weekends NO holidays NO on-call A truly exceptional schedule for a nurse seeking balance and stability.
