ChesPenn is seeking a self-motivated, skilled nurse to support the residents and preceptors who provide primary and prenatal care at our Center for Family Health at our Eastside location. This is an outstanding opportunity to be a part of a dynamic community health practice dedicated to providing the highest standard of care to everyone who needs health services. The Registered Nurse supports ChesPenn's mission to provide quality health care for all community members by coordinating healthcare services and promoting healthy behavior through patient education and care. The RN's role includes supervision of medical assistant staff.Qualifications:
RN required; BSN preferred Three (3) years in nursing or related field Current Pennsylvania Nursing Registration Current flu vaccine requiredKnowledge, Skills and Abilities:
3+ years' nursing experience, preferably in ambulatory care or community health Understanding of demographics of medically under-served communities Strong computer skills, including experience in an EMR environment Strong interpersonal skills and ability to serve as a supportive member of a health care team Possession of a valid driver's license and a licensed automobile, preferred Ability to meet approved minimal physical and medical standard Good written and verbal communication skillsWhat You'll Do:
Clinical Provide professional nursing services, including but not limited to; assessment, triage of walk-in and telephone patients, case management, planning of care and provision of care supportive to or restorative of life and well being Manage chronic and acute health problems under protocol Counsel clients on management of their illness while listening to their concerns, fears, and complaints Conduct educational sessions for all patients Administer and monitor medications, treatments, injections, immunizations, and PPD testing, as needed, to assist in the provision of care Perform EKGs, blood glucose tests, and other procedures Perform venipunctures Dress wounds Perform emergency care and routine first-aid Provide on-going patient education, counseling, follow-up, and advocacy Act as liaison for patient in respect to their care with specialists, consultants, other provider offices, laboratories, and insurance companies Refer clients needing further assessment or case management of episodic and chronic medical, surgical, podiatry, optometry, dental, psychiatric and other health problems identified through assessment Supervise and support medical assistants Provide follow-up on identified needs from quality assurance Participate in case conferences and other team activities Travel to various ChesPenn locations as requested Maintain confidentiality of clinical records and information in accordance with HIPAA regulations. Maintain a courteous and professional demeanor towards patients, staff members, and all other guests at the health center Clerical Document all patient contacts, case updates, and referrals in the medical record according to established protocols and standards Collect and organize patient data Prepare reports for various organizations requesting information on the patient's medical care Conduct chart audits for collection of clinical measures data Assist with inventories of medical supplies, medications, durable medical equipment, emergency equipment and immunizations Conduct laboratory paperwork as required Participate in meetings and committees to ensure the delivery of quality health careAdministrative:
Coordinate medical services to ensure quality delivery of care to adults, pediatrics, and women's health care patients Communicate all pertinent organizational information to reporting staff Oversees CLIA regulations Follow requirements for State and local reporting of reportable diseasesWhat We Offer:
