Job Description

We are seeking a compassionate and experienced Registered Nurse to join a growing Home Health team serving the Dickson City, PA area. This full time opportunity is ideal for nurses who enjoy working independently, building strong patient relationships, and delivering high-quality care in the comfort of patients' homes. Responsibilities:
Perform patient assessments and develop individualized care plans Provide skilled nursing care in patients' homes Educate patients and families on disease management and treatment plans Coordinate care with physicians, caregivers, and interdisciplinary team members Complete timely and accurate documentation Help patients remain safe, healthy, and independent in their homes
Experience and Skills Needed:
Active RN license Home Health experience required OASIS experience strongly preferred Strong clinical assessment and communication skills Ability to work independently and manage a field-based schedule Please apply to this posting to learn more!
Pay Details:
