Our privately owned and managed facility in the Dormont, PA area is actively seeking a dedicated Social worker to work in the social work department. In this vital role, you will oversee all social services functions, ensuring person-centered care planning, providing crucial support to residents and their families, and contributing to a warm and supportive community environment.   • Oversee and manage the day-to-day operations of the social services department, ensuring high standards of care and service delivery.
  • Develop, implement, and evaluate social service programs to meet the psychosocial needs of residents and their families.
  • Provide counseling, support, and advocacy for residents facing adjustment, emotional, or discharge planning challenges.
  • Lead and participate in interdisciplinary care planning meetings, ensuring that social service goals are integrated into comprehensive resident care plans.
  • Facilitate resident admissions, transfers, and discharges, coordinating with residents, families, and external agencies.
  • Ensure the department's compliance with all federal, state, and local regulations pertaining to social services in long-term care.
  • Supervise, mentor, and evaluate social work staff, fostering a collaborative and high-performing team.
  • Develop and maintain strong relationships with community resources to facilitate appropriate referrals and support services for residents.
  • Address and resolve resident and family concerns, promoting open communication and satisfaction. Qualifications & Experience
  • Master's Degree in Social Work (MSW) from an accredited program required.
A minimum of 3-5 years of direct social work experience in a skilled nursing facility or long-term care setting.
