Job Description

SoZen Integrative Health and Wellness is dedicated to providing comprehensive and compassionate care to mostly geriatric residents living in independent and assisted living facilities. For this position, we seek a full-time CRNP Please contact us for more details.
Salary:
$118,000.00 - $125,000.00 per year

Work Location:
Travel Between Assisted Living facilities Responsibilities Conduct comprehensive patient assessments and develop individualized care plans. Utilize EHR systems for accurate documentation and patient management. Collaborate with healthcare teams to ensure optimal patient outcomes. Educate patients and families on health management and treatment options. Stay updated on best practices in nursing and participate in clinical research as needed.
