Desired Qualifications:
Current, active RN license in Pennsylvania (or compact state eligibility) Minimum of one year of nursing experience in rehabilitation and/or long-term care BLS certification (American Heart Association or equivalent) Strong assessment, critical thinking, and communication skills Ability to adapt to fast-changing patient needs and collaborate with various healthcare team members
Responsibilities:
Administer direct patient care in accordance with individualized care plans Deliver medication, perform assessments, and document outcomes accurately Collaborate with physicians, therapists, and support staff to optimize patient recovery Educate patients and families on rehabilitation protocols and ongoing care Participate actively in quality improvement and safety initiatives Provide emotional support and advocacy for long-term care patients
