We are seeking compassionate and skilled Hospice Registered Nurses to join our dedicated team as Case Managers and provide comprehensive end-of-life care for patients and their families. Salary Range:
Full time $85,000 to $95,000; Part time $40 to $47 hourly Signing Bonus for Full time
RN:
$10,000.00 Responsibilities
- Regular assessment and reassessment of patients' needs
- Provides hospice nursing services, treatments and preventative procedures
- Observes signs and symptoms and reports to physician and IDG members any changes in patient's condition.
- Teaches, supervises and counsels the hospice patient and family members about providing care for the patient.
- Supervises, orients and trains other nursing personnel
- Coordination with the IDG and attendance at team conferences
- Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.
- Timely documentation in accordance with CMS requirements and hospice standards
- Participating in on-call rotations
- Provide emotional support and empathy in all interactions with family and staff
- Maintains skills and knowledge
- Complies with infection control policies and protocols Qualifications
- Graduated from an approved school of professional nursing
- Current RN license in Pennsylvania
- At least one year of nursing experience in hospice
- Strong clinical assessment, communication, and interpersonal skills Benefit Package for
