Requirements:

Active New York RN license Nurse Compact / Multi-state license (required) Minimum 3+ years of diabetic care experience Proven clinical experience in care coordination or case management (preferred) Must provide your own technology (computer, webcam, microphone, speakers, phone, stable internet connection) Strong communication, critical thinking, and organizational skills

Responsibilities:

Provide remote case management for diabetic patients Conduct virtual assessments and ongoing monitoring Develop, implement, and modify care plans based on patient needs Collaborate with multidisciplinary teams to optimize patient outcomes Document all interactions and updates in accordance with facility standards Ensure compliance with state regulations, professional guidelines, and organizational policies Maintain confidentiality and patient data security in a remote environment
