Job Description

Registered Nurse Assessment Coordinator
  • RNAC
  • MDS RN For over 60 years, St. Anne Home, a ministry of the Felician Sisters of North America, has been committed to offering superior health care services to the senior members of our community. We are seeking to hire a Registered Nurse Assessment Coordinator
  • RNAC to become a part of our facility's compassionate care-giving team. $3,000 Sign-On Bonus
  • $40 to $43 per hour based on experience. 3 days/week; Day Shift (Wednesdays are mandatory); All shifts are on-site. Registered Nurse Assessment Coordinator
  • RNAC Qualifications:
    Licensed as a Registered Nurse
  • RN in Pennsylvania. Minimum of 1 year of MDS experience. Minimum of 1 year of Long-Term Care experience. Minimum of 1 year of Care Plan experience. Registered Nurse Assessment Coordinator
  • RNAC Introduction:
    The RNAC assists with accurate and timely completion of both the RAI and Care Management process from admission to discharge. Registered Nurse Assessment Coordinator
RNAC Qualifications Job Summary:
Complete all required sections of the MDS. Intensive focus on
Care Plans:
creation, documentation, monitoring and leading the Care Plan meetings. Ensure documentation is present in the medical record to support MDS coding. Work in collaboration with the IDT to assess the needs and formulate a plan of care for the residents. Obtain and ensure Skilled Certification/Re-Certification documentation is accurate.
