Job Description
Job Summary:
Community Health Clinic (CHC) is seeking a compassionate, dependable, and flexible Registered Nurse (RN) - Clinical Nurse to provide clinical support across our three outpatient primary care locations. This position is primarily based in our Vandergrift and Greensburg offices , with travel to our New Kensington location as needed to support patient care, provider coverage, and operational needs. The ideal candidate enjoys working in a collaborative environment, is adaptable to changing priorities, and is committed to delivering exceptional patient care. Mileage reimbursement is provided for approved travel between CHC clinic locations.Duties and Responsibilities Patient Care & Case Management:
- Assess the care needs of established CHC patients and provide nursing case management as appropriate.
- Collaborate with CHC providers and outside healthcare professionals to coordinate comprehensive patient care.
- Obtain insurance prior authorizations for diagnostic testing, imaging, procedures, medications, and specialist referrals.
- Monitor patients requiring therapeutic drug monitoring by tracking compliance with recommended laboratory testing and follow-up care.
- Coordinate and monitor referrals to specialists, diagnostic services, and laboratory testing to ensure timely completion.
- Provide telephone triage, patient education, and follow-up care within the RN scope of practice.
- Perform patient assessments and assist providers with office procedures as needed.
Clinical Operations:
- Provide nursing coverage and clinical support at all CHC locations as assigned.
- Promote consistency in clinical workflows and patient care practices across multiple sites.
- Assist with training, mentoring, and evaluating clinical support staff on established workflows and best practices.
- Maintain accurate documentation in the electronic health record (EHR).
- Assist with quality improvement initiatives, data collection, reporting, and analysis.
- Support daily clinic operations and perform additional duties as assigned.
