SUMMARY OF POSITION
Responsible for stroke transition from inpatient to outpatient care. Works with Stroke Program coordinators, clinical providers, care coordinators, social workers, and clinic staff. Collaborates with Community Paramedicine team and Clinical RX-2-Go Pharmacist.
MINIMUM QUALIFICATION
(S): Registered nurse with a bachelor?s degree required. Minimum three (3) years of experience in nursing required. AHA BLS prior to end of orientation period. Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
PREFERRED QUALIFICATION
(S): Preferred nursing experience in neuroscience, emergency department, or quality Stroke Certification (Examples include:
ASC, NVRN, SCRN, CNRN
)
