PAY AND BENEFITS

  • 40 per hour $5,000 sign-on bonus Weekly pay with direct deposit Mileage reimbursement PTO and paid holidays Health, Dental, AFLAC insurance, Retirement Plan (company match up to 3%)
SCHEDULE
Monday through Friday 8:30 AM to 5:00 PM On-call, weekend, and holiday rotation
REGISTERED NURSE RN CASE MANAGER RESPONSIBILITIES
Manage a home health caseload in Honesdale and surrounding areas Perform RN assessments and reassessments Develop and update care plans Coordinate care with physicians and healthcare team Complete home health documentation and ensure compliance Educate patients and families
REQUIREMENTS
Active Pennsylvania Registered Nurse RN license Minimum 1 year home health RN experience Case Manager or Case Management experience preferred CPR certification Valid driver's license and reliable transportation
