Registered Nurse (On Call Hospice Nurse) Hospice 13 weeks with extension potential Guaranteed hours
Covering:
York, Adams, Cumberland, Dauphin, and portions of
Franklin County, PA Schedule:
7 on and 7 off. •Paid for 40 hours every week, even "off" week. Job Summary We are seeking a dedicated and compassionate Registered Nurse to serve as an On-Call Hospice Nurse. This vital role involves providing expert nursing care to patients in their homes or hospice facilities during on-call hours, ensuring comfort, dignity, and quality of life for individuals facing end-of-life circumstances. The ideal candidate will possess extensive clinical expertise, excellent assessment skills, and a strong commitment to compassionate patient-centered care. This position offers an opportunity to make a meaningful difference in patients' lives through flexible scheduling and collaborative teamwork. Duties Respond promptly to on-call nursing requests, providing comprehensive assessments and interventions tailored to hospice patients' needs. Administer medications, including pain management and symptom control, in accordance with physician orders and hospice protocols. Monitor patient conditions closely, utilizing vital signs, physical examinations, and diagnostic evaluation techniques to inform care decisions. Educate patients and families about disease progression, medication administration, and comfort measures to support informed decision-making. Document all patient interactions accurately within electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations. Collaborate with interdisciplinary teams—including physicians, social workers, and chaplains—to develop and update individualized care plans. Provide guidance on aseptic technique and infection control practices during procedures such as IV infusion or catheter care. Assist with discharge planning and coordinate community resources for ongoing patient support post-hospice care. Skills Proven experience in hospice & palliative medicine with a strong background in critical care or acute pain management. Proficiency in EMR systems such as Epic, Cerner, or eClinicalWorks for accurate medical documentation. Knowledge of medication administration including injections, IV infusion, and catheterization techniques. Familiarity with ICD coding (ICD-10/ICD-9) for documentation and billing purposes. Experience working with diverse populations including geriatrics, patients with developmental disabilities, memory care needs, or behavioral health challenges. Strong assessment skills with the ability to perform physical examinations and diagnostic evaluations efficiently. Excellent communication skills for triage calls, family education, and interdisciplinary collaboration. Ability to work independently during on-call hours while maintaining high standards of patient safety and care quality.
