Job Description

Gwynedd Healthcare & Rehabilitation , a premier skilled nursing and rehabilitation center in Lansdale, PA, is seeking a compassionate and detail-oriented Admissions / Desk Nurse to join our dedicated team.
Qualifications:
Current RN or LPN license in the state of Pennsylvania. Strong knowledge of state and federal healthcare regulations. Minimum of two years of experience in a Skilled Nursing Facility (SNF), preferably in short-term rehabilitation. Experience with PointClickCare electronic medical records (EMR) system is highly preferred. Excellent communication, organizational, and customer service skills.
Key Responsibilities:
Complete admission assessments and required documentation for new residents. Gather and evaluate resident information to identify individual needs and establish a baseline plan of care. Communicate effectively with residents, families, physicians, and the interdisciplinary care team to ensure a seamless admission, transfer, and discharge process. Reconcile and verify medications and treatment orders. Obtain and process physician orders for admissions. Collect resident histories and initiate appropriate follow-up actions when necessary. Secure required consents and ensure all admission documentation is completed accurately and timely. Assist with resident care conferences and interdisciplinary meetings. Provide support to Medication Cart Nurses and other nursing staff as needed. Assist the Unit Manager with administrative and clinical responsibilities. Deliver exceptional customer service to residents, families, and referral sources.
Schedule:
Full-time position Monday through Friday, 11:00 AM - 7:30 PM Participation in a rotating weekend on-call schedule may be required
