Responsibilities:

Conduct thorough initial assessments and gather necessary information from new residents, families, and healthcare providers to develop individualized care plans Coordinate the admission process, including reviewing medical records, updating resident data, and completing necessary paperwork Collaborate with the healthcare team to address resident needs, concerns, and goals, ensuring a comprehensive approach to care Develop and maintain effective relationships with residents, families, and healthcare providers to ensure a smooth transition to our community Participate in interdisciplinary team meetings to discuss resident care and progress Maintain accurate and up-to-date records, reports, and documentation Stay current with regulatory requirements, policies, and procedures, and adhere to established standards and guidelines About the
