Regional Clinical Nurse RN Job Summary We are seeking an experienced Regional Clinical Nurse RN to join our team! As a Regional Clinical Nurse RN, you will play a vital role in ensuring high-quality patient care and fostering a culture of excellence within our organization. Regional Clinical Nurse RN About-the-Role Provide clinical expertise and mentorship to nursing staff across multiple facilities, promoting evidence-based practice and best practices in patient care Conduct facility visits and assessments to identify areas for improvement and develop strategies for enhanced care delivery Ability to give clear and decisive support and oversight Collaborate with interdisciplinary teams to develop and implement care plans, addressing the unique needs of our patients Develop and maintain relationships with healthcare providers, case managers, and other stakeholders to ensure seamless transitions and continuity of care Results driven with positive clinical outcomes Regional Clinical Nurse RN Key Responsibilities Provide expert guidance and support to nursing staff, addressing questions and concerns related to patient care Conduct quality improvement initiatives, analyzing data and implementing changes to improve patient outcomes and reduce readmissions Participate in facility-wide committees, contributing to policy development and strategic planning initiatives Develop and maintain accurate and thorough documentation, ensuring compliance with regulatory requirements
Requirements License/Certification :
Current RN licensure in the state of
Pennsylvania Minimum Years of Experience :
5 years of experience, preferably in
Nursing Homes Additional Requirements :
