Job Description
Conducts Patient Visits and Assessments Performs comprehensive patient evaluations, including new patient consultations, follow-up visits, treatment assessments, and survivorship visits. Obtains and documents complete medical histories, physical examinations, treatment responses, and symptom assessments. Develops and implements evidence-based plans of care in collaboration with supervising physicians. Ensures timely patient access, thorough documentation, and appropriate follow-up care. Manages Hematology and Oncology Patients Evaluates and manages patients with hematologic and oncologic diagnoses throughout the continuum of care. Monitors disease progression, treatment effectiveness, laboratory results, imaging studies, and adverse effects. Identifies and addresses treatment-related toxicities and complications in accordance with established protocols. Collaborates with physicians regarding complex clinical issues and treatment modifications. Orders, Prescribes, and Monitors Therapies Orders and interprets diagnostic tests, laboratory studies, and imaging as clinically indicated. Prescribes medications and supportive care interventions in accordance with state regulations, institutional policies, and collaborative practice agreements. Monitors patient response to chemotherapy, immunotherapy, targeted therapy, and supportive medications. Provides symptom management for pain, nausea, fatigue, anemia, infection risk, and other cancer-related concerns. Provides Patient and Family Education Educates patients and caregivers regarding diagnoses, treatment plans, potential side effects, symptom management, and survivorship care. Reinforces treatment adherence and promotes informed decision-making. Assesses patient understanding and identifies barriers to care. Assists with Patient Triage and Acute Care Needs Participates in the assessment and triage of patient concerns, treatment-related symptoms, and urgent clinical issues. Determines appropriate interventions and level of care, including outpatient management, emergency evaluation, or hospitalization when necessary. Supports efficient clinic operations while maintaining high standards of patient safety and quality care.
