RNAC (Registered Nurse Assessment Coordinator) Havencrest Rehabilitation & Healthcare Center Join Our Team as an RNAC! Havencrest Rehabilitation & Healthcare Center is seeking an experienced and detail-oriented Registered Nurse Assessment Coordinator (RNAC) to join our clinical leadership team. Responsibilities:
Coordinate and complete the MDS assessment process in accordance with state and federal regulations. Ensure timely and accurate completion of all MDS assessments and care area assessments (CAAs). Coordinate interdisciplinary care plan meetings and maintain comprehensive care plans. Maximize Medicare and reimbursement opportunities while ensuring regulatory compliance. Review clinical documentation for accuracy and completeness. Collaborate with nursing leadership, therapy, physicians, and interdisciplinary team members. Monitor quality measures and assist with quality improvement initiatives.
Qualifications:
Current Pennsylvania Registered Nurse (RN) license in good standing. Previous
RNAC/MDS
experience in a skilled nursing facility preferred. Knowledge of RAI process, PDPM, Medicare, Medicaid, and CMS regulations. Strong organizational, communication, and computer skills.
