Job Description

Gateway Hospice, a growing hospice agency is hiring for the position of RN Case Manager for the area east of the city of Pittsburgh (Monroeville, North Huntington area). This is a full-time salaried position working Monday - Friday 8:30 a.m. to 5:00 p.m. with minimal on-call duties. Experience is preferred, however, new graduates are also welcome. Brief Description:
Responsible to the Patient Care Supervisor to provide nursing care and hospice case management to hospice patients and families. Implements nursing interventions and coordinates the Patient/Family Interdisciplinary Plan of Care. The RN Case Manager adheres to the regulatory process required by the State Board of Nursing and Federal Conditions of Participation.
Requirements for this position are:
RN Nursing License in the State of Pennsylvania Valid Drivers License with acceptable driving record. At least one year of nursing experience preferably in hospice or homecare. Nurses with Oncology, med Surg, Oncology experience also urged to apply. Involves daily travel to patients' home and facilities.
