Job Description

Job Summary The Registered Nurse Assessment Coordinator (RNAC) plays a vital role in delivering comprehensive patient assessments and coordinating care plans across diverse healthcare settings. This dynamic position involves evaluating patient needs, reviewing medical documentation, and collaborating with interdisciplinary teams to ensure optimal patient outcomes. The RNAC utilizes advanced clinical knowledge and health information systems to facilitate accurate documentation, utilization management, and discharge planning. This role is ideal for motivated nursing professionals eager to leverage their critical care, hospital, or outpatient experience in a fast-paced, impactful environment committed to excellence in patient care. Duties Conduct thorough assessments of patients' clinical conditions, utilizing physiology and anatomy knowledge to inform care decisions. Review and interpret medical records, documentation, and coding such as
ICD-10/ICD-9/ICD
coding, CPT coding, and DRG classifications to support utilization review and management processes. Collaborate with healthcare providers, case managers, and payers to develop appropriate care plans aligned with Medicare, NCQA standards, and other regulatory requirements. Utilize EMR (Electronic Medical Record) systems like Cerner, Epic, Athenahealth, eClinicalWorks, or similar platforms to document findings accurately and efficiently. Perform utilization review activities including discharge planning, medical management, and ensuring compliance with HIPAA regulations for patient privacy. Assist in clinical documentation improvement initiatives by reviewing medical records for completeness and accuracy. Support inpatient and outpatient teams across various units such as ICU, PICU, Emergency Medicine, or primary care clinics to optimize patient flow and care quality. Skills Extensive experience with EMR/EHR systems including Cerner, Epic, Athenahealth or comparable platforms. Strong understanding of managed care processes, Medicare regulations, and NCQA standards. Critical care experience in settings such as ICU or Level I/II Trauma Centers; hospital experience preferred. Knowledge of medical coding systems including
ICD-10/ICD-9/ICD
coding, CPT coding, DRG classifications for accurate documentation review. Proficiency in utilization management strategies like discharge planning and case management. Familiarity with healthcare terminology related to physiology, anatomy, nursing practices, and medical records management. Experience working within HIPAA guidelines ensuring confidentiality of medical information. Ability to navigate complex EMR systems such as Epic or Cerner efficiently while maintaining high accuracy standards. Strong communication skills for interdisciplinary collaboration across inpatient and outpatient settings including hospice care or home health environments. Background in nursing roles such as critical care nurse or medical office nurse with additional expertise in utilization review or clinical documentation improvement.
