This role is ideal for a nurse who is strong in assessment, documentation, and care planning—and who understands how to drive accurate reimbursement and quality outcomes. Key Responsibilities Complete and manage the full RAI/MDS process in accordance with federal and state guidelines. Conduct resident assessments and develop individualized care plans. Collaborate with nursing, therapy, social services, and dietary teams. Ensure compliance with
PDPM, CMS
regulations, and facility policies. Monitor deadlines, coding accuracy, and documentation requirements. Participate in quality improvement initiatives, care plan meetings, and audits. Provide education to staff on MDS processes as needed. Qualifications Active RN license in PA. Prior MDS/RAI experience in a Skilled Nursing Facility required. Strong knowledge of PDPM, care planning, and CMS regulations. Excellent documentation, analytical, and communication skills. Highly organized with strong attention to detail.
