Monday-Friday | 8:00 AM-5:00

PM On-Call:

Rotating | Limited occasional evenings/weekends

Your Role As an RN Case Manager, you are the clinical lead for your assigned patients — supported by a strong interdisciplinary team.

You will:

Conduct comprehensive nursing assessments (with Admissions Nurse support) Develop and manage individualized plans of care Collaborate with physicians, LPNs, social workers, chaplains & aides Educate families on symptom management and disease progression Participate in IDT meetings Maintain timely, compliant documentation Built to Prevent Burnout We understand hospice RN concerns.
