We're seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week). Pay and Schedule $40-$50/hour Monday-Friday, 8:00am-4:30pm On-call and weekend rotation approximately every 5 weeks Includes weekend coverage (Saturday and Sunday) Includes holiday participation (minimum one summer and one winter holiday) $6/hour hold rate plus time-and-a-half for visits during call Why This Role Stands Out Caseload of 12-14 patients (maximum 16-17) Approximately 25 visits per week (about 5 per day) 60% of visits are facility-based, supporting more efficient scheduling Centralized triage support after hours Scheduler support for daily planning Structured onboarding and training program Responsibilities Manage hospice patient caseload across home and facility settings Perform patient assessments and develop individualized care plans Provide skilled nursing care and symptom management Coordinate with interdisciplinary team members Maintain documentation using Homecare Homebase Participate in on-call rotation Territory Coverage includes Reading, Wyomissing, Sinking Spring, and Berks counties Patients are assigned geographically to support efficiency Typical day includes approximately one hour of drive time Mix of facility and home-based hospice care Benefits Mileage reimbursement Cell phone reimbursement ($40/month) Medical, dental, and vision insurance 401(k) with employer match Paid time off and holidays 4-6-week structured orientation and training Tablet, supplies, and equipment provided Requirements Active RN license At least one year of clinical experience (acute care, home health, or hospice preferred) Valid driver's license and reliable transportation Apply Today If you're seeking a hospice RN role with a manageable caseload, lower-frequency call rotation, and strong team support, we encourage you to apply to learn more. #
