RN Case Manager:

Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families. Provide hands-on care, administer medications, and evaluate patient progress. Maintain and submit timely documentation, including OASIS assessments. Collaborate with therapists, caregivers, and healthcare professionals to ensure comprehensive care. Educate patients and families on treatment plans and self-care techniques. Inform patients of additional services and resources as needed. What You Need to

Succeed:

Active RN License in Pennsylvania BLS Certification Valid Driver's License & Reliable Transportation Strong Assessment & Documentation Skills Ability to Work Independently & Manage Time Effectively Excellent Communication & Interpersonal Skills Acute Care and/or Home Health Experience (Preferred) Ready to Make an Impact?
